Family Support Referrals
Who can make a referral?
- Social Work
- Health Visitors
- Education colleagues, when the referral pathway becomes available
What is the purpose of 1:1 family support?
- To help families build confidence, recognise their strengths and access the right support when they need it.
- To offer early support that may help prevent difficulties from escalating or recurring.
- To support families who are moving on from crisis services but would still benefit from a lower level of ongoing support.
- To strengthen family and community connections within the local area.
How to make a referral
Referral partners should have a discussion with their Line Managers to discuss if referral to this service is appropriate. If so, please complete the referral form with the family’s consent. The referral will then be reviewed by the Change Manager and Hub Coordinator. They meet regularly with the relevant referral partner team manager to discuss potential referrals and agree the most appropriate next steps before allocating a Family Outreach Worker.
What happens next?
Once a referral has been allocated, a Family Outreach Worker will contact the referrer to confirm this and gather any relevant information that may help them make initial contact with the family in a supportive and informed way.
The Family Outreach Worker will contact the family using their preferred method and arrange a suitable time to meet. During the first meeting, the family will be supported to talk about what matters to them, what support they would find helpful, and how they would like this support to be provided. Together, the family and Family Outreach Worker will begin to develop a plan based on the family’s strengths, priorities and goals.
Support will be reviewed every six sessions. Reviews will continue until the family and Family Outreach Worker agree that the right support is in place and feels sustainable for the family.
Family support referrals are currently open for the Fraserburgh and Peterhead Hub. Refer a family or individual for 1:1 meeting by clicking here .

The Whole Family Wellbeing (WFW) Thematic Group is strongly underpinned by a set of national legislation and policy frameworks that promote early intervention, rights-based practice, prevention and joined-up support for families. Together, these establish both the legal duties and the practice expectations that sit behind holistic whole family support in Scotland.
